Sickle Cell Disease Treatment Demonstration Program

This notice announces the opportunity to apply for funding under the Sickle Cell Disease Treatment Demonstration Program (TDP).

The purpose of the TDP is to increase access for individuals with sickle cell disease (SCD) to quality, coordinated, comprehensive care by:
1) increasing the number

credit:


of clinicians or health professionals knowledgeable about the care of SCD, 2) improving the quality of care provided to individuals with SCD, and 3) improving care coordination with other providers.

Additional Information of Eligibility:
Eligible applicants include a Federally-qualified health center, a nonprofit hospital or clinic, or a university health center that provides primary health care, that has a collaborative agreement with a community-based sickle cell disease organization or a nonprofit entity with experience in working with individuals who have sickle cell disease; and demonstrates to the HRSA Administrator that either the federally-qualified health center, the nonprofit hospital or clinic, the university health center, the organization or entity described in clause (i) 42 USC § 300b-5, or experts as described in paragraph (2)(C) of 42 USC § 300b-5, has at least five (5) years of experience in working with individuals who have sickle cell disease.

Federally-qualified health center is defined in section 1905(l)(2)(B) of the Social Security Act (42 U.S.C.

1396d(l)(2)(B)).
